Based on the logs I just received, I know why you're not being able to login:
Google is requesting you to enter a re-captcha.
Unfortunately since this is an extremely rare occurrence, qgvdial (and its developer: me) doesn't know how to handle it.

What has worked for some other users is the following steps:
1. log out of qgvdial (click the logout button) and then the exit button
2. Open your mobile browser, go to google.com/voice and logout of there as well.
3. Re-login to google.com/voice on your phone browser, complete any re-captcha there
4. Start qgvdial and login.
