Date of entry is after maemo.org account, so I am pretty sure this is the date of garage account creation. As you will sign the form on the lower left with (the current) date.

Afaik the real name, address and the birth dates (besides the garage.maemo.org account) are mandatory.
irc and phone not.

But wait for Win7Mac to confirm.
