it's probably one Pusak put together using various utilities like queen beecon widget, ?harmatton icon pack?? theme customizer and custom operator name applet. plenty can be found in showoff your screenshot thread

there's loads in that link i posted i used to personalise my n900 as well but now generally just use themecustomizer to change aspects of themes i like font/icons set/ wallpapers.
