Are you using latest QtMobility 1.2 version?
Any error message running the widget from commandline?

"/opt/todolistwidget/bin/todolistwidget"

I have one issue:

Deleting items within the widget does not cause the widget to refresh.

The widget does not refresh if Task items are deleted in the calendar.

The widget only refreshes when a task is created by the widget.

Version 0.3.1.

It shouldn't happen. On my N900 it refreshes everytime I click on the cross and the yes button of the confirmation dialog.

Ye, it doesn't. Calendar app todo window does not refresh itself aswell if you're using todos, or if you use more than one todo widgets.
I may add autorefresh, but than any todolist widget will eat cpu to refresh itself, and qtmobility api for calendar is not so fast to retrieve data from database.
I may add an option in settings to refresh widget whenever database gets modified, but it is cpu-unfriendly.

How about updating the widget whenever you switch to its desktop, like Calendar Home Widget?
