Is it possible to swap the functionality of the back button (on facia, below task switcher button) and the menu button (on slide-out, below dpad)?

Preferably on a per-application basis, but I'd be happy with a global swap.

You can with xmodmap. The back button is Escape (keycode 9) and the menu button is F4 (keycode 70).

Just change keycode 9 to send F4 and keycode 70 to send Escape.

I've reconsidered remapping the back button globally (as I do find it useful while browsing the web).

Would xmodmap allow me to emulate a menu button press via a long press (or hold) of the full-screen button?

That would be more intuitive to me, as currently I have to press the full-screen button to access application menus via the touch-screen anyway (if I don't want to open the slider).

No, you can't do that. You can emulate any other button, including mouse clicks and double clicks, but not "long" presses.
