I believe it's Extras only, yes, not -testing and -devel.
And I believe they starting counting downloads from day 1 for Maemo 5.
It may be worth noting though that "downloads" can most probably only refer to all downloads, including the regular updates you get when you already have an application installed on your device. So if you installed only one single application from Extras and it got updated twice since then, you would be counted 3 times: 3 downloads instead of 1 application downloaded. That's technically correct and no less impressive, but it's just two different things and one has to know.
