Lets say that i install an application from extra devel or extra testing which is not optified, with result to install some files to my rootfs.
If later a new version of this application is available for download and it is optified, if i update it , it will delete the not optified files from my rootfs , or i have to unistall it first and install the new optify version

An update basically is a de-installation of the old version followed by an installtion of the new version. So yes, the non-optified files will be removed and the optified ones will be installed during an update.
