When I call up the application menu and display all the apps, I have extra icons for Clock and Media Player at the end of the list, which are duplicates.

Also, if I got to add a shortcut to the desktop, I have multiple copies of Calendar and Clock.

I'm assuming there's duplicate files somewhere, but I don't know where to look to check. Suggestions?

Ha, that's funny! I thought very few or none have experience this error for I have not seen it in any of the forms..., until now. I had this error since the the first mini update in...., Feb. I believe it was firmware PR1.1. I now have PR2.1 and have seen no improvements or corrections considering the problem. Were both in the same boot because I don't know how to fix it!
