I have no way of knowing what version it is. (like most Maemo apps, I've noticed...)

I thought I read somewhere that about boxes were discouraged, maybe I was wrong. Either way I had issues with the app menu and haven't gotten around to resolving them.
You can always go into the App.mgr, "uninstall", and see the version listed in there.
Also since you are posting a log, you can tell from that It does not look like you are using 1.0.8-6 which I just confirmed is in Fremantle extras-devel and Diablo extras.
it actually has been off and on. it just pushed me the update a few minutes ago. logs me in, but doesn't let me dial out. What is the Call Back Number field for? I haven't tried sms...

The callback number field is a required field and is your number that you want to get the ring back on when GV connects the call.
So you have login working again and now its back to the dial out issue? Want to provide an updated log?
To use the callback # field you first have to enter more than one number in the settings page of Google Voice. For example, it can be your Gizmo #, your home phone #, your cell phone #, work phone #, etc..
Then when you use dialcentral's callback settings it tells GV where to direct the call you start. So if you choose your home phone #, then a call you start from dialcentral will ring your home phone #.
Edit: The Google Voice webpage works the same way, when you type in a # or click a contact you want to call it will ask you which # (if you have more than one) to ring.

Just a small thing to add to this, Dialcentral should support custom callback numbers that do not are not associated with your account. Unfortunately I don't have extra numbers lying around to test this.
